Uses
Amiodarone is prescribed for the treatment and prophylaxis of serious cardiac arrhythmias, including ventricular tachycardia, ventricular fibrillation, atrial fibrillation and atrial flutter — particularly where other antiarrhythmic therapy is unsuitable or has not proved effective. It is a prescription-only medicine and must be initiated and monitored by a qualified physician.
How It Works
Amiodarone prolongs the cardiac action potential and refractory period, principally through blockade of potassium channels, with additional sodium-channel, beta-adrenergic and calcium-channel activity. This combined action slows abnormal electrical conduction within the heart, helping to restore and maintain a regular rhythm.
Possible Side Effects
Commonly reported effects include nausea, taste disturbance, photosensitivity and skin discolouration. Long-term therapy is associated with thyroid, hepatic, pulmonary and ophthalmic effects, which is why periodic monitoring is required. Any unusual or persistent symptoms should be reported to the treating physician.
Precautions
Amiodarone has a long elimination half-life and significant potential for drug interactions, so concurrent medication should be reviewed carefully. It is contraindicated in certain conduction disorders, severe bradycardia, thyroid dysfunction and iodine hypersensitivity. Periodic thyroid, liver, pulmonary and ophthalmic assessment is recommended during treatment. Use in pregnancy or breastfeeding only where a physician judges the benefit to outweigh the risk.
